ISONIAZID (eye soe NYE a zid) is used to prevent or to treat tuberculosis (TB).
Isoniazid 300mg, Oral tablet
Caffeine-containing Foods/Beverages
· Severity: Moderate
· Notes for Consumers: Try to limit caffeine intake while taking Isoniazid, INH. Side effects from Isoniazid, INH may get worse if you take Caffeine, like increased blood pressure, confusion, restlessness, or other side effects. All preparations containing caffeine should be used sparingly such as tea, coffee, chocolate, cola, or 'stay awake' products. Some nonprescription medicines also contain caffeine and should not be taken along with Isoniazid, INH.
· Notes for Professionals: Although isoniazid does not inhibit mitochondrial MAO, it does appear to inhibit plasma MAO. Dangerous cardiac arrhythmias or severe hypertension can occur because of the potentiation of caffeine's sympathomimetic effects by MAOIs. Caffeine use should be minimized or avoided during and for 1 to 2 weeks after discontinuation of any MAOI.

Alcohol
· Severity: Major
· Notes for Consumers: Do not drink Ethanol-containing beverages while taking Isoniazid, INH. Side effects from Isoniazid, INH can get worse. Ethanol may also decrease the actions of Isoniazid, INH by reducing the amount of INH in the body. If you develop fatigue, loss of appetite, unusual tiredness or weakness, or nausea/vomiting, contact your health care provider immediately. Also, some ethanol-based products also contain tyramine which can cause flushing, chills, or racing or pounding heartbeat if taken with Isoniazid, INH. These products include some beers (including reduced-ethanol and ethanol-free beer), red wines, sherry, hard liquor, and liqueurs. Avoid ethanol-containing beverages while taking Isoniazid, INH.
· Notes for Professionals: Daily consumption of ethanol increases the risk of isoniazid, INH-induced hepatitis and can increase the clearance of INH. Isoniazid should be discontinued if signs or symptoms of hepatitis, such as fatigue, weakness, malaise, nausea/vomiting, anorexia, or serum aminotransferase concentrations greater than three times the upper limit of normal, become evident. Isoniazid may possess enough MAO inhibiting activity to produce clinical symptoms when combined with the ingestion of tyramine-containing beverages. Some ethanol-based products also contain tyramine including some beers (including reduced-ethanol and ethanol-free beer); wines (red); sherry; hard liquor; or liqueurs. Patients should be counseled to avoid ethanol-containing beverages while taking isoniazid.

Marijuana
· Severity: Moderate
· Notes for Consumers: The effects of marijuana may be increased and side effects may get worse if it is combined with this medicine. Do not drive or operate machinery until you know how this combination will affect you. Contact your health care provider right away if you notice slurred speech, confusion, severe drowsiness, increased heart rate, or any other new or unusual side effects.
· Notes for Professionals: The incidence of marijuana associated adverse effects may change following coadministration with isoniazid, INH. Isoniazid is an inhibitor of CYP3A4, an isoenzyme partially responsible for the metabolism of marijuana's most psychoactive compound, delta-9-tetrahydrocannabinol (Delta-9-THC). When given concurrently with isoniazid, the amount of Delta-9-THC converted to the active metabolite 11-hydroxy-delta-9-tetrahydrocannabinol (11-OH-THC) may be reduced. These changes in Delta-9-THC and 11-OH-THC plasma concentrations may result in an altered marijuana adverse event profile.

Tyramine-containing foods
· Severity: Moderate
· Notes for Consumers: Food interactions with Isoniazid, INH may cause increased blood pressure or other effects. Try to limit the following foods in the diet: aged cheeses; soy sauce; fava bean or broad bean pods; smoked or pickled meats, poultry; or fish (lox, herring, smoked salmon); fermented sausages (bologna, pepperoni, salami, summer sausage); bananas; avocados; and any over-ripe fruit. Dietary restrictions should continue for at least 2 weeks after stopping Isoniazid, INH.
· Notes for Professionals: Isoniazid may possess enough MAO inhibiting activity to produce clinical symptoms when combined with the ingestion of histamine and tyramine-containing foods. Reactions reported include headache, palpitations, flushing, nausea/vomiting, and pruritus; increased blood pressure may occur if the food contains high tyramine content.
