Grapefruit juice

· Severity: Major

· Notes for Consumers: Avoid grapefruit juice and grapefruit while taking this medicine. Using this drug along with grapefruit juice can lead to an increased risk for side effects from Lemborexant. You may feel drowsy or more tired, for example. Do not drive, use machinery, or do anything that needs mental alertness after taking your nighttime dose of Lemborexant. If you notice slurred speech, confusion, disorientation, falls, sleep-walking, or hallucinations (seeing or hearing things that are not real), contact your health care provider immediately.

· Notes for Professionals: It is advisable to have patients avoid grapefruit juice and grapefruit during treatment with lemborexant due to an expected significant increase in lemborexant exposure and risk for side effects. Lemborexant is a CYP3A4 substrate; grapefruit juice is a strong CYP3A4 inhibitor. Coadministration of lemborexant with another strong CYP3A4 inhibitor increased the lemborexant AUC by up to 4.5-fold.

Alcohol

· Notes for Consumers: Avoid alcohol-containing drinks while taking this medicine. Using this drug along with alcohol can lead to an increased risk for side effects from Lemborexant. You may feel drowsy or more tired, for example. Do not drive, use machinery, or do anything that needs mental alertness after taking your nighttime dose of Lemborexant. If you notice slurred speech, confusion, disorientation, falls, sleep-walking, or hallucinations (seeing or hearing things that are not real), contact your health care provider immediately.

· Notes for Professionals: Advise patients to avoid the use of alcohol with lemborexant as concurrent use increases lemborexant exposure and the risk for additive CNS depression and impairment. Use of alcohol with lemborexant in a drug interaction study increased the lemborexant AUC by 1.5- to 2-fold. Use of lemborexant with alcohol also produced a numerically greater negative impact on postural stability and memory as compared with alcohol alone when assessed near the Tmax of lemborexant (2 hours post-dose).
